Register With The Practice

New patients wishing to register with the practice must live in our catchment area (view the map) and complete the registration form (available on-line, at reception or download - links at bottom of page).

To assist us in locating your previous medical record it would be useful if you have your NHS number.

To register with the practice we will need to see a recent (last 3 months) utility bill, bank statement or similar. The name on the documents must be the registrants name unless you can provide a marriage certificate or deed of change of name.

These documents are not a mandatory requirement and you will not be refused registration if you cannot produce them.

Please be aware that Mountwood Surgery strongly encourages parents to have their children vaccinated to protect them from serious infectious diseases. If you have reservations about vaccination, then we are here to help you. We will always continue to advocate for vaccinations for all the children under our care.

To join our practice, the link below will take you to the NHS Register with a GP Surgery page, that allows online registration with the practice, or directs you to download a paper copy of the PRF1 registration form for completion. Paper copies of the PRF1 registration form are also available from our Reception team. Please bring completed paper forms to our Reception.
